dh-make-ruby: drop exception for "ruby" in gem names

Often we will package `ruby_foo` as "ruby-foo", only to later need to
package `foo`, but "ruby-foo" is already taken. Now, `ruby_foo` becomes
"ruby-ruby-foo", so we have a less ambiguous mapping between gem names
and Debian package names.
